Specializing in preserving your dialysis access, Raleigh Access Center uses minimally invasive procedures for the critical maintenance and management of vascular accesses in an outpatient setting. The team of physicians, led by Medical Director Jeff Hoggard, MD, is dedicated to providing personalized care to every patient, delivering effective treatment to meet each patient’s individual needs.

Dr. Jeff Hoggard, interventional nephrologist and medical director, and his highly trained clinical team bring the full range of medical capabilities to every patient at every visit.

Jeff Hoggard, Interventional Nephrologist, Medical Director

Jeff Hoggard, MD

Jeffrey Hoggard, MD, FACP, FASN, FASDIN, is an interventional nephrologist specializing in dialysis access management. Taking a minimally invasive approach, Dr. Hoggard performs a range of procedures, including stent placement, balloon angioplasty, thrombectomy and thrombolysis, to maintain and manage vascular accesses for dialysis patients. Dr. Hoggard is the Medical Director of Raleigh Access Center in Raleigh, North Carolina.

Dr. Hoggard earned his medical degree from the University of North Carolina at Chapel Hill School of Medicine. He completed his residency in internal medicine and a fellowship in nephrology at the University of Texas Health Science Center in San Antonio.

Dr. Hoggard co-authored numerous articles and is a fellow of the American College of Physicians, the American Society of Nephrology and the American Society of Diagnostic and Interventional Nephrology.

Dr. Hoggard is board certified in internal medicine and nephrology. He is also certified by the American Society of Diagnostic and Interventional Nephrology in hemodialysis vascular access procedures.

James Godwin

James Godwin, MD

James Godwin, MD, received an undergraduate degree from the University of Kansas. He continued at Kansas for medical school followed by training in Internal Medicine, Nephrology at the University of North Carolina at Chapel Hill.

Sejan Patel

Sejan Patel, MD, FACP, FASN, FASDIN

Sejan B. Patel, MD, received Nephrology training through Duke University Hospitals and Internal Medicine training through Case Western Reserve University Hospitals. He graduated from The Ohio State University College of Medicine and University of Michigan. He has been certified by the American Society of Diagnostic and Interventional Nephrology. Areas of specialization include interventional nephrology, where he performs endovascular interventions on dialysis access, and home dialysis therapy.

Adam Stern

Adam Stern, MD

Adam Stern, MD, received his undergraduate degree in Biology from Hofstra University and his Doctor of Medicine from New York Medical College. He then completed his Internal Medicine Residency and Nephrology Fellowship at UNC Chapel Hill.

Dorian DeFrietas

Dorian DeFreitas, MD

Dr. Dorian DeFreitas, MD, is a vascular surgeon specializing in dialysis access management and surgical access creation. He performs critical maintenance and management of vascular accesses for dialysis patients. Dr. DeFreitas treats patients at Raleigh Access Center, using the most up-to-date, minimally invasive techniques to help patients improve their quality of life.

Dr. DeFreitas earned his medical degree from the National University of Ireland, in Galway, Ireland. He went on to complete his residency in General Surgery at East Carolina University, in Greenville, NC, followed by a fellowship in Vascular Surgery and Endovascular Surgery at Emory University in Atlanta, Georgia.

Dr. DeFreitas is board certified in Vascular Surgery.

Akhil Hegde

Akhil Hegde, MD

Dr. Hegde, MD, received an undergraduate degree at the University of North Carolina at Pembroke and is a graduate of the Brody School of Medicine at East Carolina University. He completed his Internal Medicine training and Chief Residency at Wake Forest Baptist Medical Center, followed by Nephrology Fellowship at UNC Hospitals. Dr. Hegde is certified by the American Society of Diagnostic and Interventional Nephrology in hemodialysis vascular access procedures.

Kawan Swain

Kawan Swain, MD

Dr. Swain, MD, is a graduate of the Brody School of Medicine at East Carolina University. He completed his Internal Medicine training and Chief Residency at Wake Forest Baptist Medical Center, followed by Nephrology Fellowship at UNC Hospitals. Dr. Swain is also a certified Interventional Nephrologist, focusing on evaluation and management of hemodialysis vascular access.

Jason Kim, MD

Dr. Jason Kim, MD, is a vascular surgeon specializing in dialysis access management and surgical access creation. He performs critical maintenance and management of vascular accesses for dialysis patients. Dr. Kim treats patients at Raleigh Access Center, using the most up-to-date, minimally invasive techniques to help patients improve their quality of life.

Dr. Kim earned his medical degree from Medical College of Georgia, Augusta, GA. He went on to complete his residency in General Surgery at Ochsner Clinic, New Orleans, LA, followed by a fellowship in Vascular Surgery and Endovascular Surgery at Ochsner Clinic, New Orleans, LA.

Dr. Kim is board certified in Vascular Surgery.
